Rather than scrambling around trying to manage each crisis as it peeks its head, we’ve created a membership-based Geriatric Care Management practice truly designed around our clients’ needs. We make it faster, easier and more relaxing to address issues effecting seniors. Remotely, we are able to do a lot more than you think we could, and if you need hands on help, we can help coordinate that for you too.

 

With an ECN membership you can interact as much, or as little as you want to. We are here to help you with issues effecting your aging parent, no matter where they live in the United States. Our model is relationship based. We work hard to build a trusting relationship with you and your family members. We are reliable, dependable and honest. We want your loved one to have the best care possible. We are able to address a broad range of issues related to the well-being of our clients, while balancing this with our extensive knowledge about the costs, quality and availability of resources in your community.

 

When you join our subscription-based plan, it allows you to hop-on and hop-off of our services anytime you want, while giving you the flexibility to try our services. We always put our customers first, which allows you to pay a small amount to try out our service, instead of a significant upfront amount for something which you may not even use after a while. Along with increasing access to perinate solutions to questions regarding seniors needs, our subscriptions create a more economical, reliable and sustainable solution for our clients.

 

The potential benefits are similar to those of many consumer-focused subscriptions. You will receive personalized information to help you better understand your needs, so you can make more informed choices and cost-focused decisions.

 

We can research skilled nursing facilities, long term care facilities, Assisted Living Facilities, Elder Law Attorneys, Therapists, Fiduciaries, Home Health Agencies, Wealth Managers, Housekeepers, Handymen, Skilled Rehabilitation Facilities, Assisted Living Facilities, other resources and licensed experts. We can give you recommendations and make introductions whether you need a quick answer to a simple question or more detailed research. We do not receive any kickbacks or compensation from anyone we refer you to, which unfortunately is uncommon in this industry.

 

Our Subscriptions allow clients to access Geriatric Care Management services at affordable rates that you may not have access to. 

 

We can help:

  • Research alternative living environments, including Skilled Nursing Facilities, Long Term Care Nursing Homes, Independent Living Facilities and Continuing Care Retirement Communities.

  • We can refer you to Estate Planning or Elder Law Attorneys in your area.

  • We can locate and organize Home Health for you under your insurance.

  • In some cases, help you apply for Medicaid.

  • Give you recommendations to make your home safer and set up a home safety evaluation at your house if you desire.

  • Recommend meal delivery services, grocery delivery programs in your area.

  • Help you work with your long-term care insurance.

  • We can talk with nurses, social workers and physicians on your behalf during hospital, nursing home and other in-patient stays to act as an advocate and liaison in your care to make sure you fully understand your stay and subsequent next steps.

  • We are available every day and will respond within 24 hours, more frequently, the same day. 

 

 

What we can’t do:

  • Our provided assistances is done remotely, from time to time you may need hands on assistance, which we can set up for you, but it is not included in the plan. Think of us as a resource in providing information, researching vendors for you, helping you understand your insurance, setting up appointments etcetera.

  • If we are unable to help you, we can refer you to a local specialist (nurse, attorney, Home Care, etcetera). The expense of other professionals we refer you to, are not included in our plan. We do not accept any payment or kickback from anyone we refer you to. We attempt to vet professionals who we think will do a good job for you and meet your needs. If you ever have a bad experience with someone, we refer you to, we fully expect that you notify us, so we can attempt to help resolve and/or potentially not refer them to any other client.

  • Sometimes when applying for Medicaid, you may need the assistance of an Elder Law attorney, we can help make introductions. Again, referrals to outside specialist are not covered under our plan.

 

Our goal is to create loyalty and we think you will be happy with the knowledge base of our navigators. If we do not have the answer, we will research it for you and/or refer you a specialist in your community. We are different than most services, who refer vendors to you, because they are paid by the vendors to do such. Again, we receive no compensation from any service we refer you to. We recognize that you are our customers, not them.
